PRACTICUM APPROVAL FORM

To ensure a high quality academic experience, students interested in enrolling in a practicum

(1) at an independent placement (i.e. not a School-Designed Practicum);

(2) at a placement where they have previously worked (either volunteer, for credit or paid) in a prior summer or academic semester and/or

(3) at a for-profit entity must obtain approval from Dean Ramji-Nogales prior to
registering.

Please submit the completed form before noon on April 24, 2025 for the Summer 2025 semester; before noon on July 24, 2025 for the Fall 2025 semester; and before noon on December 11, 2025 for the Spring 2026 semester.

Are you related by birth or marriage to your supervising attorney or any attorney at this placement?  If so, please list the names of the attorneys at the placement with whom you have a familial relationship.

Please describe the attorney supervision structure at the placement. Your answer should be detailed and precise, and must include information such as the name and role of the attorney who will supervise you, how often you will receive feedback from that attorney, and what form that feedback will take (written, oral, one-on-one meetings, etc.)

Please list existing experiential opportunities, including internal clinics, external clinics, externship courses, and school designed practicums, that are similar to your proposed practicum, and explain in detail why these experiential offerings are not sufficient to meet your learning goals
