PRACTICUM APPROVAL FORM

To ensure a high quality academic experience, students interested in enrolling in a practicum must obtain approval from Dean Lee prior to registering if they are seeking an independent placement (i.e. not a School-Designed Practicum). Please read the instructions on how to fill out the form prior to completing your application. Even if you have previously worked at this placement, you must fill out this form to receive academic credit for each semester in which you expect to receive credit.

Your form will not be considered if it is submitted after the deadline. You should expect to hear back approximately one week after submission.
