PRACTICUM APPROVAL FORM

To ensure a high quality academic experience, students interested in enrolling in a practicum

(1) at an independent placement (i.e. not a School-Designed Practicum);

(2) at a placement where they have previously worked (either volunteer, for credit or paid) in a prior summer or academic semester and/or

(3) at a for-profit entity must obtain approval from Dean Ramji-Nogales prior to
registering.

Please submit the completed form before noon on April 25, 2024 for the Summer 2024 semester; before noon on July 25, 2024 for the Fall 2024 semester; and before noon on December 12, 2024 for the Spring 2025 semester.
