PRACTICUM APPROVAL FORM

To ensure a high quality academic experience, students interested in enrolling in a practicum must obtain approval from Dean Lee prior to registering if they are seeking:

(1) an independent placement (i.e. not a School-Designed Practicum);

(2) a placement where they have previously worked (either volunteer, for credit or paid) in a prior summer or academic semester; and/or

(3) a placement with a for-profit entity.

Please submit the completed form before noon on April 24, 2025 for the Summer 2025 semester; before noon on July 24, 2025 for the Fall 2025 semester; and before noon on December 11, 2025 for the Spring 2026 semester. You should expect to hear back approximately one week after submission.
